Cyclosporine Capsules Patient Assistance Program – For Those Who Don’t Have Insurance
If you don’t have prescription insurance coverage from either private or public you may qualify for the Cyclosporine Capsules Patient Assistance Program.
This program is there to help those without insurance afford their prescription medication, Cyclosporine Capsules. To qualify for this program you must:
Not have nor be eligible for private or public insurance
Be a resident of the U.S, Puerto Rico, or the U.S. Virgin Islands
Have a valid RX for any of the TEVA medications on the application (ProAir HFA)
Have a total family annual income of 200%—or less—of the federal poverty level for your state
How to Enroll in the Teva Patient Assistance Program
To sign up for the Cyclosporine Capsules Patient Assistance Program you must fill out an application. When you have filled out the required information you must take the application to your healthcare provider. Your healthcare provider then in turn should follow the instructions on the application. You will receive an approval or denial by mail. Once approved your healthcare provider will receive your medication through the mail and should provide it to you. After approval you are eligible to recieve assistance for Proair for up to 12 months.
